Payment Policies
Payment for counseling is due at the time of service. I accept cash, personal checks, and credit/debit/HSA cards. An itemized bill detailing the services received will be provided. This is called a “superbill” and can be submitted to insurance companies and other third-party payer to request reimbursement.
It is strongly recommended to new clients that contact their insurance providers to determine their policy’s out-of-network provider benefits. While this direct payment system requires extra work for our clients, it enhances our ability to protect health information and allows us to keep our fees reasonable.
Cancellations/No Show Policy
If a scheduled appointment must be canceled or rescheduled, please provide 24-hour notice. Headwaters will allow one No Show appointment without charge. Thereafter, unless emergency or unforeseeable situations arise, clients will be charged a fee for No Shows. The fee is $50.
